One type of form for GENERAL and OBC candidates and the other one for SC/ST/PWD candidates. Although in both the forms the information to be filled is exactly same except that the category is written on both the forms.
For General and OBC students the offline form cost Rs 100 which includes prospectus as well as an Application form.

For SC/ST/PWD the Application for will cost Rs 50/-which includes Prospectus + Application form.

Note: Before Submitting the offline forms don't forget to keep a photocopy of the filled form. It will used at the time of taking admission in college.
